Symposium Registration
Registration deadline is Monday, January 16.
Registration fee includes tuition for all sessions, course material, meals and snacks served before, during and after sessions. Individual Specialty Track options are also available.
The following options are available to expedite your symposium registration:
Register online at BaptistHealth.net/cme.
Mail registration with payment to the Baptist Health CME Department, 8900 North Kendall Drive, Miami, Florida 33176.
Fax the registration form with your credit card information to 786-533-9706.
Confirmations will be sent to acknowledge registrations received before Monday, January 16, 2012.
Registrations will not be processed or confirmed without full payment. Rates increase $50 after January 16, 2012.
Cancellation Policy
Cancellations must be sent in writing and received or postmarked by Monday, January 16, 2012 to receive a refund of the registration fee, less a $25 administrative fee. Cancellations received or postmarked after January 16 will forfeit the registration fee.
