**Registration must be accompanied by a letter from the Fellowship/Residency Director.
The registration fee includes tuition for the entire symposium, continental breakfast on Friday and Saturday and lunch on Friday.
Confirmations will be sent to acknowledge registrations received before October 5. Registrations will not be processed or confirmed without full payment.
